Introduction to a generalized method for adaptive randomization in trials

چکیده
BACKGROUND Ideally clinical trials should use some form of randomization for allocating participants to the treatment groups under trial. As an integral part of the process of assessing the effectiveness of these treatment groups, randomization performed well can reduce, if not eliminate, some forms of bias that can be evident in non-randomized trials. Given the vast set of possible randomization methods to choose from we demonstrate a method that incorporates many of the advantages of these other methods. METHODS A step-by-step introduction of how to use the adaptive randomization algorithm for conducting a clinical trial is given. RESULTS The implications, effects and capabilities of using the adaptive randomization algorithm are fully demonstrated and explained using simulated data and examples from actual trials. CONCLUSIONS This paper provides an introduction to a dynamic type of treatment allocation, which fulfills the CONSORT requirements of participants being randomly allocated whilst maintaining a level of control of the balances overall, within the stratification variables and within the strata simultaneously. Maintaining control of the imbalances within the groups is vital particularly if interim analyses are planned. TRIAL REGISTRATION Current controlled trials ISRCTN17551624, ISRCTN37558856, ISRCTN97185214.

متن کاملEstimating the Optimal Dosage of Sodium Valproate in Idiopathic Generalized Epilepsy with Adaptive Neuro-Fuzzy Inference System
Introduction: Epilepsy is a clinical syndrome in which seizures have a tendency to recur. Sodium valproate is the most effective drug in the treatment of all types of generalized seizures. Finding the optimal dosage (the lowest effective dose) of sodium valproate is a real challenge to all neurologists. In this study, a new approach based on Adaptive Neuro-Fuzzy Inference System (ANFIS) was pre...
متن کاملA simulation study of outcome adaptive randomization in multi-arm clinical trials.
Randomizing patients among treatments with equal probabilities in clinical trials is the established method to obtain unbiased comparisons. In recent years, motivated by ethical considerations, many authors have proposed outcome adaptive randomization, wherein the randomization probabilities are unbalanced, based on interim data, to favor treatment arms having more favorable outcomes. While the...
متن کاملAN ADAPTIVE WAVELET SOLUTION TO GENERALIZED STOKES PROBLEM
In this paper we will present an adaptive wavelet scheme to solvethe generalized Stokes problem. Using divergence free wavelets, theproblem is transformed into an equivalent matrix vector system, thatleads to a positive definite system of reduced size for thevelocity. This system is solved iteratively, where the applicationof the infinite stiffness matrix, that is sufficiently compressible,is r...
متن کاملAdaptive Randomization Using PROC MCMC
Based on work by Thall et al. (2012), we implemented a Bayesian method for randomizing patients in a Phase II trial. According to this method, each patient is rated for efficacy and toxicity levels at a specified time point after being randomized to a dose. Evidence is accumulated that identifies which dose(s) of a cancer treatment provide the most desirable profile, per a utility score matrix ...
